Case Manager

Responsibilities

• Provide individual case management and oversight to TAY youth, including home visits and weekly check-ins
• Conduct intake, assessment, and trauma screening
• Provide housing navigation and resource brokerage
• Coordinate and advocate for youth within the agency and with other CBOs
• Collect data and enter case notes into appropriate databases
• Design individual support and transition plans focusing on independent living skills and income stability
• Attend community, city, and regional housing meetings

Requirements

• Bachelor's degree in a related field or equivalent work experience
• 2-3 years of experience working with high-risk and homeless youth
• Valid California Driver's License and daily access to a car
• Experience working with diverse staff and clients
• Proficiency in Google Suite and Microsoft Office

Preferred Qualifications

• Experience using the ONE/HMIS System
• Experience with US Department of Housing and Urban Development (HUD) programs
• Experience with San Francisco HSH policies
• Bilingual in Spanish
