About the role
Responsibilities
- Conduct Youth Coordinated Entry intake and offer problem-solving assistance for immediate housing solutions
- Provide case coordination and advocacy within the agency and with other community-based organizations
- Design and implement individual support and transition plans for youth
- Maintain records in assigned databases and perform follow-up/aftercare with rehoused clients
- Develop and implement policies for the Youth Access Point and maintain the access office
- Provide immediate support to youth including hygiene supplies and food
- Attend community, city, and regional housing meetings
Requirements
- Bachelor's degree in a related field or equivalent work experience
- 2-3 years of experience working with high-risk youth experiencing housing crises
- Valid California Driver’s License with a clean driving record and daily access to a car
- Proficiency in Google Suite
- Ability to provide substance abuse counseling
- Strong organizational and multitasking skills
Preferred Qualifications
- Experience using the ONE System
- Experience with US Department of Housing and Urban Development (HUD) programs
- Bilingual in Spanish
