Case Manager

Responsibilities

• Conduct Youth Coordinated Entry intake and offer problem-solving assistance for immediate housing solutions
• Provide case coordination and advocacy within the agency and with other community-based organizations
• Design and implement individual support and transition plans for youth
• Maintain records in assigned databases and perform follow-up/aftercare with rehoused clients
• Develop and implement policies for the Youth Access Point and maintain the access office
• Provide immediate support to youth including hygiene supplies and food
• Attend community, city, and regional housing meetings

Requirements

• Bachelor's degree in a related field or equivalent work experience
• 2-3 years of experience working with high-risk youth experiencing housing crises
• Valid California Driver’s License with a clean driving record and daily access to a car
• Proficiency in Google Suite
• Ability to provide substance abuse counseling
• Strong organizational and multitasking skills

Preferred Qualifications

• Experience using the ONE System
• Experience with US Department of Housing and Urban Development (HUD) programs
• Bilingual in Spanish
