About the role
Responsibilities
- Provide comprehensive case management to Transitional Aged Youth (TAY) placed into housing via Coordinated Entry
- Conduct twice monthly home visits and in-person visits for individual case oversight
- Create housing retention plans focused on self-sufficiency and independent living skills
- Perform resource brokerage and referrals for behavioral health, employment, education, and legal services
- Manage detailed client records, physical files, and data entry in HMIS for program compliance
- Support Housing Program Manager and Navigators in all aspects of the Rapid Rehousing program
- Attend community, city, and regional housing meetings
Requirements
- Bachelor's degree in a related field or equivalent work experience
- 2-3 years of experience working with high-risk and homeless youth
- Valid California Driver's License and clean driving record with daily access to a car
- Proficiency in Microsoft Office (Excel and Word) and Google Suite
- Ability to work in a multicultural environment and build rapport with youth
Preferred Qualifications
- Experience using ONE, Clarity, or HMIS systems
- Experience with US Department of Housing and Urban Development (HUD) programs
- Bilingual in Spanish
