Case Manager

Responsibilities

• Provide comprehensive case management to Transitional Aged Youth (TAY) placed into housing via Coordinated Entry
• Conduct twice monthly home visits and in-person visits for individual case oversight
• Create housing retention plans focused on self-sufficiency and independent living skills
• Perform resource brokerage and referrals for behavioral health, employment, education, and legal services
• Manage detailed client records, physical files, and data entry in HMIS for program compliance
• Support Housing Program Manager and Navigators in all aspects of the Rapid Rehousing program
• Attend community, city, and regional housing meetings

Requirements

• Bachelor's degree in a related field or equivalent work experience
• 2-3 years of experience working with high-risk and homeless youth
• Valid California Driver's License and clean driving record with daily access to a car
• Proficiency in Microsoft Office (Excel and Word) and Google Suite
• Ability to work in a multicultural environment and build rapport with youth

Preferred Qualifications

• Experience using ONE, Clarity, or HMIS systems
• Experience with US Department of Housing and Urban Development (HUD) programs
• Bilingual in Spanish
