About the role
Responsibilities
- Create Individual Service Plans (ISP) and attendance logs for housing, mental health, and employment
- Connect youth to wrap-around services and provide warm hand-offs to providers
- Perform resource brokerage and referrals for behavioral health, education, and legal services
- Assist youth in achieving ISP goals and creating housing retention plans
- Conduct service assessments and pre-and-post surveys
- Manage data collection and case notes in appropriate databases
- Support the Housing Program Manager with barrier removal programs
- Attend community, city, and regional housing meetings
Requirements
- Bachelor's degree in a related field or equivalent experience
- 2-3 years of experience working with high-risk and unhoused youth
- Proficiency in Microsoft Office (Excel and Word) and data management systems
- Valid California Driver's License with a clean driving record and daily access to a car
- Strong organizational skills and ability to manage high volumes of paperwork
Preferred Qualifications
- Experience using ONE, Clarity, HMIS, or GMS systems
- Experience with HUD or MOHCD programs
- Proficiency in Google Suite
- Bilingual in Spanish
