Case Manager

Responsibilities

• Create Individual Service Plans (ISP) and attendance logs for housing, mental health, and employment
• Connect youth to wrap-around services and provide warm hand-offs to providers
• Perform resource brokerage and referrals for behavioral health, education, and legal services
• Assist youth in achieving ISP goals and creating housing retention plans
• Conduct service assessments and pre-and-post surveys
• Manage data collection and case notes in appropriate databases
• Support the Housing Program Manager with barrier removal programs
• Attend community, city, and regional housing meetings

Requirements

• Bachelor's degree in a related field or equivalent experience
• 2-3 years of experience working with high-risk and unhoused youth
• Proficiency in Microsoft Office (Excel and Word) and data management systems
• Valid California Driver's License with a clean driving record and daily access to a car
• Strong organizational skills and ability to manage high volumes of paperwork

Preferred Qualifications

• Experience using ONE, Clarity, HMIS, or GMS systems
• Experience with HUD or MOHCD programs
• Proficiency in Google Suite
• Bilingual in Spanish
