Case Management Nurse II

Responsibilities

• Support the physician and interdisciplinary team in providing patient care to enhance clinical outcomes and patient satisfaction
• Integrate and coordinate utilization management, transitions of care, and discharge planning functions
• Manage a designated patient caseload to ensure appropriate care settings and efficient resource utilization
• Collaborate with multidisciplinary care teams, ancillary departments, and post-acute care providers
• Utilize Electronic Health Records for accurate documentation of assessments, referrals, and care variances
• Provide age-specific and culturally competent discharge planning and patient/family education

Requirements

• Associate degree in Nursing
• Registered Nurse License
• BCLS or ACLS Certification
• 5 years of Nursing Clinical experience
• 1 year of recent Acute Case Management experience (or comparable experience such as clinical liaison or navigator)
