About the role

Responsibilities

  • Conduct client assessments and develop personalized care plans in collaboration with clients, representatives, and stakeholders
  • Perform regular home visits to monitor clinical risks and ensure client needs are being met
  • Update and review care plans frequently to adapt to changing client requirements
  • Coordinate with internal teams, including Rostering and Practice Leads, to ensure seamless service delivery
  • Develop tailored strategies to address the specific needs of Aboriginal and Torres Strait Islander clients
  • Represent the organization at external forums and community meetings to promote aged care services

Requirements

  • Demonstrated experience in Aged Care case management
  • Strong understanding of Aged Care funding and service delivery, specifically Support at Home
  • Proficiency in Microsoft Office (Outlook, etc.) and experience using database systems and mobile devices
  • Excellent communication skills and the ability to build relationships with diverse stakeholders
  • Ability to work autonomously and multi-task in a fast-paced environment
  • Current unrestricted Australian Driver's Licence and a reliable vehicle for regional travel
