Care Manager

Responsibilities

• Provide high-quality hands-on care and physical and emotional support to residents based on their Individualized Service Plans (ISP).
• Observe, report, and document changes in resident conditions, including skin, behavior, alertness, and diet.
• Assist residents with activities of daily living (ADLs) and encourage participation in life enrichment activities.
• Respond promptly to resident call bells and security systems.
• Maintain a safe, clean, and comfortable home-like environment, including assisting with light housekeeping and laundry as needed.
• Support dining services by serving meals and ensuring residents follow their prescribed diets.
• Document daily care logs and participate in team meetings to ensure continuity of care.

Requirements

• High School diploma or GED.
• Valid CPR and First Aid certification.
• Must be at least 18 years of age.
• Strong communication skills and the ability to remain calm in difficult situations.
• Ability to handle multiple priorities and demonstrate good judgment.

Preferred Qualifications

• Previous experience working with seniors in an assisted living or long-term care setting.
• A strong desire to serve and care for the elderly population.
