About the role
Responsibilities
- Manage and deliver direct nursing care to participants in clinics, adult day centers, homes, and SNFs
- Serve as the "quarterback" for a participant panel within the Interdisciplinary Team (IDT)
- Conduct face-to-face nursing assessments covering physical, psychosocial, and behavioral statuses
- Deliver personalized care and document nursing interventions in partnership with medical providers
- Coordinate complex patient discharges, transfers, and post-discharge needs
- Manage medication, medical equipment, supplies, and specialist referrals
- Triage in outpatient settings and initiate therapies within scope of practice
- Educate participants, caregivers, and family members on care plans
- Perform clinical interventions including wound care, IVs, and phlebotomy
- Delegate tasks to MAs and LVNs
- Participate in a rotating after-hours call schedule
Requirements
- Graduate of an accredited school of nursing
- Unencumbered California Registered Nurse (RN) license
- 2-4 years of clinical experience with medically complex or older adult populations
- 1-2 years of case management experience
- Strong clinical acumen in chronic disease management and geriatric care
- Experience with wound care, IVs, phlebotomy, and colostomy/ileostomy care
- Current CPR/BLS certification (or ability to obtain within 30 days)
- Valid CA driver’s license, personal transportation, and good driving record
- Medical clearance for communicable diseases and up-to-date immunizations
Preferred Qualifications
- Alzheimer’s certification
- Case Management certification
- Bilingual skills in Spanish, Mandarin, or Cantonese
