Care Coordinator

Responsibilities

• Interact routinely with clients and communicate significant observations to clinical staff
• Monitor client progress on goals and assist with programming revisions
• Assist clients with scheduling community appointments and educational needs
• Provide facilitation and co-facilitation of approved group sessions
• Oversee safe transport of clients to and from facilities
• Assist with treatment planning, discharge planning, and new admissions orientation
• Utilize de-escalation skills to maintain safety for clients and staff
• Document client care, behavior, and group sessions in accordance with policies

Requirements

• Bachelor’s Degree in a relevant field or commensurate experience
• At least 21 years of age
• 1+ year of relevant work experience
• Strong interpersonal and communication skills
• Ability to implement and interpret departmental programs and policies
• Knowledge of the surrounding geographic area for efficient routing
• Ability to organize and prioritize competing demands

Preferred Qualifications

• Experience with de-escalation techniques
• Proficiency with Microsoft Office Suite
• Strong organizational and problem-solving abilities
• Ability to handle confidential material reliably
