Care Coordinator

Responsibilities

• Interact with adolescent clients, observe behaviors, and communicate significant observations to clinical staff
• Monitor client progress on goals and collaborate with counseling and educational staff to adjust programming
• Assist clients with scheduling community appointments and educational needs
• Co-facilitate approved group sessions and assist with treatment and discharge planning
• Oversee safe transport of clients and maintain route and vehicle records
• Assist with orientation of new admissions and respond to emergency situations
• Utilize de-escalation techniques to maintain safety and document care per established policies

Requirements

• Bachelor's degree in a relevant field or equivalent experience
• Must be at least 21 years of age
• 1+ year of relevant work experience
• Knowledge of the surrounding geographic area for effective routing
• Strong interpersonal, communication, and organizational skills
• Ability to prioritize competing demands and maintain professional boundaries

Preferred Qualifications

• De-escalation techniques
• Proficiency with Microsoft Office Suite
• Excellent written and oral communication skills
• Creative and persistent problem solving abilities
• Ability to multi-task in a fast-paced environment
