About the role
Responsibilities
- Serve as a Wraparound Facilitator/Care Coordinator within the System of Care Fast Forward program
- Work with an integrated team to provide therapeutic services to youth and families experiencing Severe Emotional Disturbance
- Support children and youth at risk for acute psychiatric hospitalization or out-of-home placement
- Coordinate care and services to help families thrive within their communities
Requirements
- Associates degree or better in Social Work or a related field
- Ability to travel extensively within the state of New Hampshire
- Flexibility to work a schedule that includes evenings and weekends based on family needs
Preferred Qualifications
- Master’s degree in Social Work or a related field
- Bachelor’s or Associate’s degree in human services combined with four years of relevant experience
- Experience working with juvenile justice or other at-risk juvenile populations
