Care Coordinator

Responsibilities

• Serve as a Wraparound Facilitator/Care Coordinator within the System of Care Fast Forward program
• Work with an integrated team to provide therapeutic services to youth and families experiencing Severe Emotional Disturbance
• Support children and youth at risk for acute psychiatric hospitalization or out-of-home placement
• Coordinate care and services to help families thrive within their communities

Requirements

• Associates degree or better in Social Work or a related field
• Ability to travel extensively within the state of New Hampshire
• Flexibility to work a schedule that includes evenings and weekends based on family needs

Preferred Qualifications

• Master’s degree in Social Work or a related field
• Bachelor’s or Associate’s degree in human services combined with four years of relevant experience
• Experience working with juvenile justice or other at-risk juvenile populations
