Care Coordinator

Responsibilities

• Conduct client assessments and develop personalized care plans with clients and stakeholders
• Regularly update care plans and perform home visits to ensure high-quality, person-centred care
• Monitor clinical risks and client needs, ensuring appropriate referral pathways are established
• Collaborate with internal teams, including Rostering and Practice Leads, to meet client requirements
• Develop strategies to address the specific needs of Aboriginal and Torres Strait Islander clients
• Represent the organization at external forums and community meetings

Requirements

• Demonstrated experience in Aged Care clinical case management
• Strong understanding of Aged Care funding and service delivery, specifically Support at Home
• Proficiency in Microsoft Office, Outlook, and database systems
• Ability to work autonomously and multi-task in a fast-paced environment
• Current unrestricted Australian Driver's Licence and a reliable vehicle
• Strong communication skills and the ability to build relationships with diverse stakeholders
