Care Coordinator Lead

Responsibilities

• Supervise and mentor a team of Care Coordinators, including onboarding and training
• Serve as the first point of escalation for referrals, prior authorizations, and clinical workflows
• Coordinate daily huddles and team coverage schedules
• Process and route incoming referrals, lab orders, and home health orders
• Manage relationships with external home health agencies and partner laboratories
• Coordinate DME orders and maintain accurate patient records in EHR
• Triage patient calls and interface with patient intake and billing teams
• Relay clinical messages between providers and patients/families

Requirements

• Active RN license in the State of Florida (compact/multistate accepted)
• 3+ years of care coordination, case management, or clinical operations experience
• 1+ year of team lead or supervisory experience in a healthcare setting
• Experience with electronic health records (EHR/EMR) systems
• Proficiency with Microsoft Teams, Outlook, and standard office applications
• Strong knowledge of Medicare/Medicaid billing and home health workflows

Preferred Qualifications

• Experience in home health, primary care, or mobile healthcare delivery
• Bilingual English/Spanish
• Familiarity with prior authorization processes and Cover My Meds
• Experience with lab ordering platforms
• BSN preferred
