Care Coordinator - Home Care

Responsibilities

• Support older Australians in Western Sydney by coordinating essential services to promote independence.
• Build and maintain strong relationships with clients, their families, and referrers.
• Conduct client assessments and develop personalized care plans tailored to individual needs.
• Act as a trusted point of contact throughout the client's Home Care journey.
• Work closely with internal teams to ensure exceptional service delivery and compliance.
• Manage multiple priorities while maintaining accurate documentation.

Requirements

• Experience in home care, aged care, or community services.
• Experience in support coordination, case management, or healthcare settings.
• Proven background in client-facing customer service.
• Strong organizational skills and the ability to manage multiple priorities.
• Excellent communication skills and a customer-first mindset.
• Knowledge of Home Care or Support at Home services is highly regarded.
