About the role

Responsibilities

  • Conduct thorough assessments of guest needs, strengths, and challenges to develop individualized care plans.
  • Connect guests with essential resources including housing programs, mental health services, substance use treatment, and job training.
  • Advocate for guests' rights and needs within the shelter and the broader community by coordinating with social service agencies and government entities.
  • Provide immediate support and intervention during crisis situations, such as mental health crises or medical emergencies.
  • Monitor guest progress, update care plans, and maintain accurate, up-to-date documentation in compliance with organizational policies.
  • Collaborate with shelter staff and external service providers to ensure coordinated and holistic care.
  • Provide follow-up support services for one month after guests are successfully transitioned into permanent housing.

Requirements

  • High school diploma or equivalent required.
  • Minimum of two years of care coordination or case management experience.
  • Minimum of two years of paid experience supporting unhoused populations with multiple disabilities.
  • Proficiency in Homeless Management Information System (HMIS).
  • Proficiency in Microsoft Office, Google Suite, and Zoom.
  • Demonstrated experience and cultural sensitivity in working with individuals facing substance use disorders or mental health challenges.

Preferred Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field.
  • Strong knowledge of trauma-informed care, cultural competency, and client-centered approaches.
