About the role
Responsibilities
- Conduct thorough assessments of guest needs, strengths, and challenges to develop individualized care plans.
- Connect guests with essential resources including housing programs, mental health services, substance use treatment, and job training.
- Advocate for guests' rights and needs within the shelter and the broader community by coordinating with social service agencies and government entities.
- Provide immediate support and intervention during crisis situations, such as mental health crises or medical emergencies.
- Monitor guest progress, update care plans, and maintain accurate, up-to-date documentation in compliance with organizational policies.
- Collaborate with case managers, shelter staff, and external providers to ensure holistic and coordinated care.
- Provide follow-up support services for one month after guests are successfully transitioned into permanent housing.
Requirements
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field (or an equivalent combination of education and experience).
- Minimum of two years of care coordination or case management experience.
- Minimum of two years of paid experience supporting unhoused populations with multiple disabilities.
- Proficiency in HMIS, Zoom, Microsoft Office, and Google Suite.
- Ability to travel to multiple sites within San Francisco County.
- Demonstrated experience and cultural sensitivity working with individuals facing substance use disorders or mental health challenges.
