Care Coordinator

Responsibilities

• Conduct scheduled and unscheduled in-person drive-bys on potential home health patients who are non-responsive to phone contact
• Facilitate start-of-care (SOC) processes by confirming patient presence and readiness for evaluation
• Serve as a liaison between the patient and the home health care team to reduce gaps in care
• Assist in verifying patient demographic and insurance information at point of contact
• Communicate findings in real time to the SOC Operations Team, Clinical Manager, and Sales Team
• Perform basic assessments within EMT-B scope of practice to identify immediate safety concerns
• Coordinate with Clinical Manager when a patient requires emergency intervention
• Maintain confidentiality and comply with all HIPAA regulations

Requirements

• Current, active Texas EMT-Basic certification
• Valid Texas Driver's License with valid auto insurance
• Reliable personal or agency-provided vehicle
• Strong written and verbal communication skills
• Ability to work independently with minimal supervision

Preferred Qualifications

• Prior experience in home health, community paramedicine, or EMS
• Bilingual (English/Spanish)
• CPR/BLS certification (American Heart Association)
