Care Coordinator

Responsibilities

• Coordinate patient-centered care activities using the EMR system
• Support providers and clinical teams through accurate documentation and follow-up tracking
• Serve as a communication link between providers and internal care teams
• Communicate patient needs and urgent concerns to the clinical team
• Assist with care transitions, referrals, and coordination with external partners
• Maintain accurate, timely documentation in accordance with established workflows
• Contribute to team collaboration and day-to-day office operations

Requirements

• Legally authorized to work in the United States
• Active, unrestricted clinical certification or licensure (PCT, CNA, MA, CMA, LPN, RN, or comparable)
• Minimum 1 year of experience in a clinical/administrative or care coordination role
• Proficiency with Microsoft 365 (Outlook, Word, Excel)
• Strong knowledge of healthcare terminology
• Ability to work onsite in downtown Tulsa, OK, Monday through Friday, 8:30 a.m. to 5:00 p.m. CST
• Ability to pass a background check and drug screen
