About the role

Responsibilities

  • Provide on-site engagement with dialysis patients to support care plans and transitions
  • Coordinate patient care using EMR to ensure continuity and quality outcomes
  • Schedule patient visits for providers and manage daily clinic workflow
  • Collaborate with Ennoble Care providers, dialysis facility staff, and multidisciplinary teams
  • Serve as a liaison between providers, patients, families, and facility staff
  • Communicate patient needs, changes in condition, and safety concerns to clinical leaders
  • Identify and resolve barriers impacting patient care coordination
  • Maintain accurate and timely documentation in accordance with healthcare regulations

Requirements

  • Active, unrestricted clinical certification or licensure (PCT, CNA, MA/CMA, LPN, or RN)
  • Valid, unencumbered driver’s license and reliable transportation
  • Proficiency in Microsoft 365 (Outlook, Word, Excel)
  • Ability to work on-site Monday through Friday, 5:00 a.m. to 1:00 p.m. CST
  • Successful completion of background check and drug screen

Preferred Qualifications

  • Dialysis-specific certification (CDT, CHT, CCHT, CCHT-A)
  • Experience managing provider schedules and patient appointments in a clinical setting
  • Minimum one year of experience working with hemodialysis/nephrology patients
