Care Coordinator

Responsibilities

• Coordinate patient-centered care activities using the EMR system
• Support providers and clinical teams through accurate documentation and follow-up tracking
• Serve as a communication link between providers and internal care teams regarding clinical updates
• Communicate patient needs and urgent concerns to the clinical team promptly
• Assist with care transitions, referrals, and coordination with external partners
• Maintain accurate documentation in accordance with established workflows and escalation protocols

Requirements

• Active, unrestricted clinical certification or licensure (PCT, CNA, MA/CMA, LPN, or RN)
• Minimum 1 year of experience in a clinical/administrative or care coordination role
• Proficiency with Microsoft 365 (Outlook, Word, Excel)
• Strong knowledge of healthcare terminology and care coordination principles
• Ability to work onsite in the College Park area of Atlanta, GA
• Ability to pass a background check
