Care Coordinator, Embedded

Responsibilities

• Coordinate care for long-term care residents using Ennoble Care’s EMR
• Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
• Collaborate closely with Ennoble Care providers within the long-term care setting
• Serve as a key liaison between providers, facility staff, residents, and families
• Communicate patient needs, changes in condition, and urgent concerns to the clinical team
• Accurately document patient interactions and care coordination activities
• Follow established workflows, documentation standards, and escalation protocols

Requirements

• Active, unrestricted clinical certification or licensure (PCT, CNA, MA, CMA, LPN, or RN)
• Minimum 1 year of patient-facing experience in long-term care or skilled nursing
• Proficiency with Microsoft 365 (Outlook, Word, Excel)
• Valid, unencumbered driver’s license and reliable transportation
• Ability to work on-site in the Atlanta, GA area (focus on Statesboro)
• Ability to pass a background check and drug screen
