Care Coordinator, Durable Medical Equipment

Responsibilities

• Complete individualized patient care plans and perform care coordination using the electronic medical record system
• Maintain frequent patient contact to provide support and manage compliance with care management programs
• Document all client communications accurately and in a timely manner
• Communicate with Primary Care Clinicians regarding patient emergent needs and ensure care plans are complete
• Collaborate with cross-organizational teams to meet patient, family, and partner facility needs
• Adhere to documentation protocols for daily work logs, escalations, and internal communications

Requirements

• Comfortable speaking on the phone for extended periods
• Compassionate and empathetic with exceptional customer service skills
• Proficient in Microsoft Office, Outlook, and general computer use
• Knowledge of basic healthcare terms, conditions, and care principles
• Ability to multitask, remain organized, and thrive in a fast-paced environment
• Must work full-time on-site in Marlton, NJ, Monday through Friday, 8:30am–5:00pm
• Able to pass a drug screen and background check
