Care Coordinator and Discharge Planner

Responsibilities

• Coordinate and manage the discharge process for residents to ensure seamless transitions to next levels of care
• Develop and implement comprehensive discharge plans in compliance with regulatory standards
• Arrange post-discharge services including home health care, outpatient therapy, and community resources
• Communicate discharge plans effectively to residents, families, and staff
• Maintain accurate documentation of discharge planning activities
• Ensure compliance with state and federal regulations, including Medicare and Medicaid
• Provide education and support to residents and families regarding care options
• Report patient health concerns to Registered Nurses or Medical Providers

Requirements

• Minimum 2 years of experience in discharge planning, case management, or related healthcare role
• Current, full, and unrestricted LPN/LVN license in the state of Florida
• Bachelor's degree in Social Work, Human Services, or a related field, OR graduate of an accredited nursing program
• Basic Life Support (BLS) certification
• Valid driver's license
• Ability to respond to disaster notification within 24 hours and establish care within 72 hours
• Ability to work onsite in austere environments and extreme weather conditions
• Proficiency in Microsoft Office and electronic health records

Preferred Qualifications

• Experience working with special needs populations
• Knowledge of Medicare and Medicaid regulations
