About the role

Responsibilities

  • Support individuals with planned activities and community involvement to promote social inclusion
  • Develop relationship maps to identify opportunities for community engagement
  • Provide continuous supervision and intervention to maintain safety and well-being
  • Assist individuals with personal care, hygiene, and daily living skill development
  • Participate in Person-Centered Planning and implement individual goals
  • Complete required documentation, including data collection and progress notes
  • Administer medications in accordance with agency policies and regulations
  • Report any safety, health, or human rights concerns immediately

Requirements

  • High School Diploma or GED
  • Valid driver's license
  • Reliable transportation
  • Proof of automobile insurance
  • Ability to lift up to 50 lbs and perform physical tasks such as walking, standing, and pushing wheelchairs

Preferred Qualifications

  • Previous experience in human services or working with individuals with developmental disabilities
