About the role
Responsibilities
- Support individuals with planned activities and community engagement to promote social inclusion
- Develop relationship maps to identify social inclusion opportunities and foster meaningful connections
- Provide supervision and intervention to ensure safety within the community
- Assist with individualized supports, including personal care, hygiene, bathing, and toileting
- Participate in person-centered planning and implement individual goals
- Complete required documentation, data collection, and progress notes
- Administer medications in accordance with state regulations and agency protocols
- Report any safety, health, or human rights concerns immediately
Requirements
- High School Diploma or GED
- Valid driver's license and a reliable vehicle for travel with supported individuals
- Proof of automobile insurance
- Ability to lift up to 50 lbs and perform physical tasks such as walking, standing, and transferring
- Ability to remain calm and respond appropriately in escalated or stressful situations
Preferred Qualifications
- Previous experience in human services or a related field supporting individuals with disabilities
