About the role
Responsibilities
- Build rapport and engage with individuals regarding their life goals and barriers to recovery
- Drive to and meet with individuals in their homes and various community locations
- Collaborate with individuals and the team to develop, implement, and review person-centered treatment plans
- Help individuals develop strategies for success in living, learning, working, and socializing
- Maintain accurate and current case documentation within an electronic health record
- Support individuals in identifying and utilizing community resources and supports
Requirements
- Bachelor's degree in psychology, social work, human services, or a related field
- OR Certified Peer Specialist (CPS) certification
- OR CPRP (Certified Psych Rehab Practitioner) certification
- Valid US driver's license held for at least two years
- Access to a reliable vehicle with valid registration and inspection
- Valid auto insurance and ability to provide proof of coverage
- Ability to meet physical demands including walking, climbing stairs, and assisting individuals in the community
Preferred Qualifications
- Prior work or internship experience related to mental health services
