Behavioral Health Case Manager

Responsibilities

• Provide non-intensive case management services to support the treatment goals of Behavioral Health patients
• Serve as a member of a multi-disciplinary patient care team
• Engage in quality improvement and quality assurance activities for the department
• Foster a person-centered approach that maximizes patient self-determination and respects cultural needs
• Maintain detailed and accurate documentation within electronic health records
• Advocate for patient needs while maintaining professional boundaries and confidentiality

Requirements

• Bachelor's degree in social work or a related behavioral health field
• Two years of relevant work experience
• Valid and unrestricted Arizona driver's license
• Ability to pass a criminal background check and obtain a Class I Fingerprint Clearance Card
• Basic Life Support (BLS) certification (must be obtained within 90 days of hire)
• Ability to pass substance abuse testing

Preferred Qualifications

• Master's degree in social work or a related field
• Bilingual in Spanish
• Experience working with underserved and diverse populations
