Behavioral Health Care Manager

Responsibilities

• Integrate behavioral health care into primary care settings
• Support individuals experiencing mental health and substance use concerns
• Provide brief, evidence-based interventions
• Manage a defined patient caseload
• Conduct screenings and track treatment progress
• Coordinate with physicians, psychiatrists, and other behavioral health providers
• Support care transitions and connect patients with resources

Requirements

• Formal education or specialized training in behavioral health (social work, nursing, or psychology)
• Minimum 2 years of social work or behavioral health experience
• Experience screening for common mental health and substance use disorders
• Reliable transportation, valid driver’s license, and valid automobile insurance
• Ability to travel daily within the service area to patient homes, assisted living, or nursing homes

Preferred Qualifications

• Working knowledge of differential diagnosis for mental health and substance use disorders
• Experience with assessment and treatment planning
• Proficiency in evidence-based psychosocial treatments
• Strong therapeutic engagement and rapport-building skills
• Basic knowledge of psychopharmacology within scope of practice
