About the role

Responsibilities

  • Act as a community connector by performing community mapping to promote social inclusion for individuals with developmental disabilities
  • Assist individuals with planned activities, community involvement, and developing a network of social connections
  • Provide individualized support, including personal care, skill acquisition, and assistance with daily functional living skills
  • Maintain a calm demeanor and respond appropriately to aggressive or verbal outbursts using approved de-escalation techniques
  • Complete required documentation, including scheduled data collection and monthly progress notes
  • Assist in developing Person Centered Planning by providing information for assessments and implementing goal strategies
  • Distribute medication in accordance with applicable regulations

Requirements

  • High School Diploma or GED
  • MOAB Day 1, MANDT, or formal de-escalation training
  • Valid driver's license and reliable transportation
  • Proof of automobile insurance
  • Ability to lift up to 50 lbs and assist with physical transfers
  • Strong personal boundaries and professional communication skills
