About the role

Responsibilities

  • Conduct participant intakes and manage assigned caseloads for pregnant participants and families
  • Provide ongoing case management and connect participants to community resources to address social determinants of health
  • Deliver social-emotional and maternal health education aligned with the Babies To Be curriculum
  • Execute daily program activities, including maintaining accurate records and data entry
  • Participate in community outreach, health fairs, and recruitment activities to increase program visibility
  • Provide direct supervision and mentorship to Social Work students, ensuring they meet academic and training requirements

Requirements

  • Bachelor’s degree from an accredited program
  • 2 years of related experience in case management, program coordination, maternal health, or community-based services
  • Must meet one of the following professional criteria:
  • Licensed Social Worker (LISW-S with supervisor designation) or ability to acquire Ohio licensure within 6 months
  • Bachelor of Science in Nursing and Registered Nurse (RN)
  • Master’s degree in Counseling
  • Demonstrated ability to supervise others

Preferred Qualifications

  • Master's degree from an accredited program with licensure in Ohio
  • Familiarity with community resources in Montgomery County for pregnant mothers and their families
  • Previous experience in a supervisory role
  • Strong team player mentality
