About the role
Responsibilities
- Serve as the attending physician for patients in assigned skilled nursing facilities
- Collaborate with NPs/PAs to manage daily patient care
- Provide patient-centered primary care, diagnosing and treating acute and chronic conditions
- Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and quality initiatives
Requirements
- MD or DO with active U.S. medical license
- Board-certified or board-eligible in an appropriate specialty
- Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine
