About the role
Responsibilities
- Serve as the attending physician for patients in assigned skilled nursing facilities
- Collaborate with NPs and PAs to manage daily patient care
- Diagnose and treat acute and chronic conditions with a patient-centered approach
- Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and quality initiatives
Requirements
- MD or DO with an active U.S. medical license
- Board-certified or board-eligible in an appropriate specialty
- New graduates who are board-eligible and actively pursuing licensing are encouraged to apply
