APRN/PA - Palliative Care

Responsibilities

• Provide palliative medicine consultative services and family-centered care for patients in long-term care facilities and hospice settings.
• Obtain patient health histories and perform physical examinations.
• Communicate effectively with patients and families regarding goals of care, advance care planning, and end-of-life transitions.
• Prescribe and manage medications related to palliative and hospice pharmaceuticals.
• Act as a member of the Interdisciplinary Team (IDT), collaborating with physicians, nurses, social workers, and chaplains.
• Document all patient encounters accurately to meet documentation and billing standards.
• Provide education to patients, families, and healthcare professionals.
• Evaluate patient progress and current standards of care to ensure optimal quality of life.

Requirements

• Master's degree in Advanced Practice Nursing.
• Current New Hampshire Advanced Practice Registered Nurse (APRN) license.
• Current New Hampshire Registered Nurse (RN) license.
• Valid DEA license.
• At least one year of experience in palliative care or hospice settings.
