Application Analyst I - Epic Healthy Planet

Responsibilities

• Design, build, and maintain Epic Healthy Planet and Compass Rose components to support patient coordination workflows
• Configure care coordination tools including registries, care gaps, outreach workflows, and patient tracking
• Support ServiceNow ticket queues by troubleshooting and resolving end-user issues efficiently
• Analyze complex population health and care coordination needs to translate them into technical Epic solutions
• Collaborate with clinical, operational, and IT stakeholders to optimize care management workflows
• Document workflows, configurations, and system updates to ensure ongoing optimization and compliance
• Identify system gaps in patient coordination processes and recommend scalable enhancements

Requirements

• Active Epic certification in Healthy Planet, Compass Rose, or another relevant Epic application
• Experience supporting patient coordination, care management, or population health workflows
• Strong background in business process analysis and system design within healthcare environments
• Ability to work independently and manage high-volume work environments with minimal supervision
• Effective communication skills to engage clinicians, analysts, and operational stakeholders

Preferred Qualifications

• Knowledge of healthcare IT standards and data exchange principles (e.g., HL7)
• Understanding of care coordination models, quality measures, and patient engagement strategies
