BScPhm or PharmD, Certificate of Qualification from Pharmacy Examining Board of Canada, Registration with Ontario College of Pharmacists Part A, 3 years hospital practice preferred, PGY1 residency preferred

About the role

Responsibilities

  • Act as an antimicrobial stewardship resource for internal and external healthcare professionals
  • Collaborate with interprofessional teams, including Infectious Disease, Microbiology, and Infection Prevention & Control
  • Lead antimicrobial stewardship patient care rounds and provide audit and feedback to clinical teams
  • Develop, implement, and evaluate evidence-based best practice empiric treatment algorithm guidelines
  • Provide teaching on antimicrobial pharmacotherapy to medical, pharmacy, and nursing students and learners
  • Perform prospective and retrospective evaluations of antimicrobial prescribing practices
  • Establish initiatives to improve antimicrobial use, lower resistance, and increase patient safety and cost-efficiency

Requirements

  • BScPhm or PharmD from an accredited program
  • Certificate of Qualification from the Pharmacy Examining Board of Canada
  • Registration with the Ontario College of Pharmacists (Part A)
  • 3 years of hospital practice preferred
  • PGY1 residency preferred

Preferred Qualifications

  • Completion of a PharmD degree
  • Certificate from the Making a Difference in Infectious Diseases (MAD-ID) Program
  • PGY2 residency or fellowship with specialization in Antimicrobial Stewardship or Infectious Diseases
  • Post-graduate pharmacy degree (e.g., Master's degree)
  • Knowledge of quality improvement, implementation science, and research methods
