Annual Wellness Visit RN

Responsibilities

• Conduct thorough reviews of medical and family histories
• Evaluate current medications and supplements for potential risks
• Measure and record vital signs including blood pressure, heart rate, and weight
• Screen for health risks such as depression, cognitive impairment, and fall risks
• Collaborate with patients to create individualized prevention plans
• Provide patient education on lifestyle factors like diet, exercise, and tobacco cessation
• Work closely with supervising physicians to ensure accuracy and quality of care
• Facilitate referrals to specialists and document findings in EMR
• Monitor and track patient progress in preventative care programs

Requirements

• Graduate of an accredited nursing program
• Current and unrestricted RN license in the state of employment
• Minimum of 2 years of nursing experience, preferably in primary care, geriatrics, or preventative health
• Proficient in electronic medical records (EMR)
• Current and valid CPR certification

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Familiarity with current Annual Wellness Visit guidelines and requirements
