About the role
Responsibilities
- Provide high quality in-home primary and palliative care to patients in the Atlanta, GA area
- Assess patient health through physical examinations and medical history updates
- Develop differential diagnoses and comprehensive treatment plans
- Communicate effectively with office staff and clinical operations teams
- Consult with patients, families, and caregivers to foster trust
- Order diagnostic testing and prescribe necessary medications and supplies
Requirements
- Board Certified Nurse Practitioner (NP) or Physician Assistant (PA)
- Valid NP or PA certification in the State of Georgia
- 1-2 years of primary care experience with geriatric patients
- Strong clinical diagnostic and assessment skills
- Ability to problem solve confidently

- Hospice care experience
