About the role

Responsibilities

  • Provide high quality in-home primary and palliative care to patients in the Atlanta, GA area
  • Assess patient health through physical examinations and medical history updates
  • Develop differential diagnoses and comprehensive treatment plans
  • Communicate effectively with office staff and clinical operations teams
  • Consult with patients, families, and caregivers to foster trust
  • Order diagnostic testing and prescribe necessary medications and supplies

Requirements

  • Board Certified Nurse Practitioner (NP) or Physician Assistant (PA)
  • Valid NP or PA certification in the State of Georgia
  • 1-2 years of primary care experience with geriatric patients
  • Strong clinical diagnostic and assessment skills
  • Ability to problem solve confidently

Preferred Qualifications

  • Hospice care experience
