Advanced Practice Provider - SNF

Responsibilities

• Provide primary and palliative care to patients in a skilled nursing facility
• Perform physical examinations and assess patient health
• Develop impressions, diagnoses, and treatment plans
• Order diagnostic testing and prescribe medications as needed
• Communicate with office staff and clinical operations
• Attend quarterly medical staff meetings
• Foster trust with patients, families, and caregivers during visits

Requirements

• Board Certified NP or PA with valid state certification
• Strong clinical skills to accurately assess, diagnose, and treat
• Ability to confidently problem solve and operate independently
• Positive demeanor and strong interpersonal skills

Preferred Qualifications

• 1-2 years of primary care experience with geriatric patients
• Hospice or palliative care experience
