Advanced Practice Provider, Nurse Practitioner, Physician Assistant - Advanced Heart Failure

Responsibilities

• Manage comprehensive direct patient care for critically ill patients in the hospital setting, specifically within the Advanced Heart Failure department.
• Utilize advanced assessment skills to evaluate, diagnose, and manage patients with complex conditions such as valve disease, coronary artery disease, and those requiring heart transplants or LVAD placement.
• Order and interpret diagnostic tests, perform therapeutic interventions, and prescribe pharmacological agents.
• Develop and implement individualized plans of care in collaboration with physicians, nursing staff, and multidisciplinary teams.
• Monitor patient progress, maintain continuity of care through follow-up, and adjust treatment plans as necessary.
• Provide education and support to patients and their families regarding diagnoses and care management.

Requirements

• Advanced degree from an accredited nursing program (for NP) or completion of an accredited Physician Assistant program (for PA).
• Current Registered Nurse license and ability to obtain Illinois APN license by start date (for NP).
• State of Illinois licensure/registration (for PA).
• AANC certification (ACNP/ANP-BC) for Nurse Practitioners or NCCPA certification for Physician Assistants.
• ACLS certification.
• ICU certification.

Preferred Qualifications

• Minimum of two years of critical care clinical nursing experience.
• Demonstrated competence in managing patients with critical illness and knowledge of critical care protocol therapy.
• Previous ICU experience.
