About the role
About the Company
Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Clinicians go to the home of the patient, providing a continuum of care for those with chronic conditions and limited mobility through programs like remote patient monitoring and chronic care management.
Responsibilities
- Provide high quality care to primary and palliative patients in and around Washington County
- Assess patient health via physical examinations, medical histories, and care planning
- Communicate effectively with office staff and clinical operations
- Develop impressions, diagnoses, and treatment plans
- Foster trust with patients, families, and caregivers
- Order diagnostic testing and prescribe medications or necessary interventions
Requirements
- Board Certified Nurse Practitioner (NP) or Physician Assistant (PA)
- Valid NP or PA certification in the state of Maryland
- 1-2 years of primary care experience with geriatric patients
- Strong clinical, problem-solving, and independent operation skills
- Hospice experience is highly preferred
