About the role

About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Clinicians go to the home of the patient, providing a continuum of care for those with chronic conditions and limited mobility through programs like remote patient monitoring and chronic care management.

Responsibilities

  • Provide high quality care to primary and palliative patients in and around Washington County
  • Assess patient health via physical examinations, medical histories, and care planning
  • Communicate effectively with office staff and clinical operations
  • Develop impressions, diagnoses, and treatment plans
  • Foster trust with patients, families, and caregivers
  • Order diagnostic testing and prescribe medications or necessary interventions

Requirements

  • Board Certified Nurse Practitioner (NP) or Physician Assistant (PA)
  • Valid NP or PA certification in the state of Maryland
  • 1-2 years of primary care experience with geriatric patients
  • Strong clinical, problem-solving, and independent operation skills
  • Hospice experience is highly preferred
