Advanced Practice Provider

Responsibilities

• Provide primary and palliative care to homebound patients in Salem County, NJ
• Perform physical examinations and obtain, update, and study medical histories
• Develop impressions, diagnoses, and treatment plans for patients with chronic diseases
• Order diagnostic testing and prescribe medications and medically necessary supplies
• Communicate effectively with office staff and clinical operations across internal platforms
• Foster trust and consult with patients, families, and caregivers during visits
• Attend quarterly medical staff meetings

Requirements

• Board Certified Nurse Practitioner or Physician Assistant with valid New Jersey certification
• 1-2 years of primary care experience with geriatric patients
• Strong clinical skills with ability to accurately assess, diagnose, and treat
• Ability to problem solve and operate independently in the field
• Positive demeanor and strong interpersonal skills

Preferred Qualifications

• Hospice or palliative care experience
