About the role
Responsibilities
- Complete complex MCA assessments and apply adult‑care legislation to young adults aged 16–25 transitioning within the 0–25 Disabilities Service
- Undertake DoLS and BIA‑related work, including preparing documentation for authorisations and ensuring compliance with statutory frameworks
- Develop and review adult‑focused support plans that promote independence, safeguard wellbeing and ensure smooth transition into adult services
Requirements
