About the role
Responsibilities
- Manage an adult caseload, performing assessments and care management tasks
- Participate in a general duty rota under the supervision of the Team Manager
- Conduct and coordinate assessments using Personal Outcome Planning tools
- Develop and implement care packages for adults and young people transitioning to Adult Services
- Oversee care management to ensure packages remain effective and responsive to changing needs
- Support the Team Manager in delivering high-quality services and efficient care plan implementation
Requirements
- SSSC registration
- Degree-level qualification in Social Work
- Post-qualified experience
- Eligibility to work in the UK
