About the role

Responsibilities

  • Assist individuals and families with developmental disabilities through education, support, and resource referral to achieve personal goals and community integration.
  • Conduct home visits and provide information/referral assistance for accessing benefits and community resources.
  • Engage families in person-centered planning to identify short-term and long-term goals and necessary community connections.
  • Facilitate Individual Service Plan (ISP) development, including coordinating meetings and ensuring necessary evaluations are completed.
  • Act as a liaison between service providers, individuals, families, and the agency to monitor and evaluate service provision.
  • Advocate for the legal, civil, and human rights of individuals while encouraging self-determination and choice-making.
  • Maintain accurate documentation, including progress notes, MIS database updates, and compliance with state regulations.

Requirements

  • Associate's Degree required.
  • Two years of experience in human services, developmental services, special education, or a related field.
  • Valid NH driver's license and an acceptable driving record.
  • Access to a personal vehicle for frequent travel and home visits.
  • Ability to communicate effectively in both verbal and written formats.
