About the role
Responsibilities
- Provide direct support to families through home visits, information referrals, and assistance in accessing benefits.
- Engage families in person-centered planning to identify short and long-term goals and community connections.
- Facilitate Individual Service Plan (ISP) development, including coordinating meetings and ensuring goal alignment.
- Act as a liaison between service providers, individuals, families, and the agency to monitor and evaluate service provision.
- Advocate for the legal, civil, and human rights of individuals with developmental disabilities.
- Maintain accurate documentation, including progress notes, MIS database updates, and compliance with state regulations.
- Identify and educate the community to eliminate stereotypes regarding persons with disabilities.
Requirements
- Associate's Degree required.
- Two years of experience in human services, developmental services, special education, or a related field.
- Valid New Hampshire driver's license with an acceptable driving record.
- Access to a personal vehicle for frequent travel and home visits.
- Ability to work a flexible schedule and handle occasional high-stress situations.
