About the role
About the Company
Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Clinicians provide a continuum of care in the home for patients with chronic conditions and limited mobility, offering programs like remote patient monitoring and behavioral health management.
Responsibilities
- Assess patients for appropriate criteria for hospice admission
- Evaluate physical, psychological, social, and spiritual needs of patients and families
- Collaborate with physicians and the Interdisciplinary Group to establish and implement plans of care
- Coordinate medications, supplies, and DME
- Maintain communication with the Medical Director, Team Director, and other agencies
- Document patient records and ensure plans of care reflect current and ongoing status
Requirements
- Associate's Degree required; Bachelor's Degree preferred
- Licensed Registered Nurse (RN) in the state of Georgia
- Previous experience in hospice, home health, or general medical-surgical nursing
- Reliable automobile, valid driver's license, and required auto insurance
