About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Clinicians provide a continuum of care in the home for patients with chronic conditions and limited mobility, offering programs like remote patient monitoring and behavioral health management.

Responsibilities

• Assess patients for appropriate criteria for hospice admission
• Evaluate physical, psychological, social, and spiritual needs of patients and families
• Collaborate with physicians and the Interdisciplinary Group to establish and implement plans of care
• Coordinate medications, supplies, and DME
• Maintain communication with the Medical Director, Team Director, and other agencies
• Document patient records and ensure plans of care reflect current and ongoing status

Requirements

• Associate's Degree required; Bachelor's Degree preferred
• Licensed Registered Nurse (RN) in the state of Georgia
• Previous experience in hospice, home health, or general medical-surgical nursing
• Reliable automobile, valid driver's license, and required auto insurance
