Admissions Coordinator

Responsibilities

• Coordinate safe and efficient transitions between care settings for incoming residents and patients
• Manage census and patient throughput to achieve and exceed growth targets
• Support application throughput and data entry across EMR, Allscripts, Curaspan, and Referral Manager PRO
• Provide customer service support for Short-Term and Long-Term Care inquiries via phone and email
• Conduct unit tours and respond to applicant inquiries professionally
• Communicate with Case Managers to ensure smooth transitions from hospital to skilled nursing care
• Review Long-Term Care applications for completeness and appropriateness
• Verify insurance eligibility, benefits, and obtain necessary authorizations
• Establish and maintain relationships with referral sources, physicians, and hospitals
• Perform screenings including Megan’s Law, immunizations, and PASSR reviews
• Welcome and orient newly admitted residents to their rooms

Requirements

• High school diploma or equivalent (AA, AS, BA, or BS preferred)
• 1-2 years of Skilled Nursing Facility (SNF) admissions experience
• Proficiency in Microsoft Office (Outlook, Word, Excel)
• Ability to work in a fast-paced environment with urgency and professionalism
• Strong interpersonal and organizational skills
• Ability to lift, push, and pull a minimum of 25 pounds

Preferred Qualifications

• Skilled nursing billing and insurance experience
