About the Company

For over 125 years, Calvary Hospital and Calvary @ Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care.

Responsibilities

• Visit patients referred for admission to Calvary @ Home and assess for eligibility
• Inform patients, caregivers, and family members of available hospice services
• Communicate patient status with the respective interdisciplinary team
• Develop strong relationships with hospitals, physicians, and nursing facilities through outreach and networking
• Provide education to patients and caregivers about hospice care
• Work with physicians and interdisciplinary teams to develop initial plans of care
• Generate and respond to referrals from hospitals and community partners

Requirements

• Current nursing license to practice in the state of service
• Valid driver’s license and access to a vehicle (except for Manhattan-only roles)
• 2 years of experience in an acute care setting
• Graduate from an accredited school of nursing (RN, BSN preferred)

Preferred Qualifications

• 1 year of hospice experience
