Abdominal Transplant Clinical Pharmacist

Responsibilities

• Provide pharmaceutical care and clinical pharmacy services for patients within the abdominal transplant population
• Collaborate with an interprofessional team of physicians, nurses, and advanced practice providers on therapeutic plans
• Review and assess medication profiles to ensure drugs are safe and effective
• Obtain medication histories and provide drug information to patients and healthcare providers
• Educate patients on proper medication use, including cautions, adverse reactions, and potential interactions
• Oversee pharmacy technicians to facilitate insurance authorizations and medication preparation as needed

Requirements

• Doctor of Pharmacy degree (PharmD) or equivalent
• Two or more years of experience in a hospital setting (or equivalent combination of education and experience)
• Current California Pharmacy License in good standing
• Current Basic Life Support (BLS) certification from the American Heart Association

Preferred Qualifications

• Completion of an ASHP-accredited PGY1 Pharmacy Residency
• At least 3 years of inpatient or acute care services pharmacy experience
• Advanced practice board certification (e.g., BPS, AAHIVP)
• Experience participating in pediatric and adult emergency situations
